Fibromyalgia
A patient presents with fatigue, feels more tired in the morning on waking than going to bed, memory loss, difficulty concentrating, pain in the neck, back and shoulders. On further questioning she has nausea, sometimes bloating. She has a history of frequent bladder infections and has been shown to have had a lab test from the States show she has Lyme disease.
Ideally the forms / questionnaires for fibromyalgia should not have Fibromyalgia mentioned anywhere on the form. Patients do not appreciate the diagnosis. Not until it has been explained to them in a non-judgmental, this is not in your head way, which is why I use a white-out on the Fibromyalgia Form
